The new technology helps to pinpoint disease at earlier stages and provides doctors more information for planning appropriate treatment. The 64-slice CT can also be used for many different tests including CT Angiography, vascular studies, cardiac imaging, neurological exams, and 3-D reconstructions of various organs in the body. The new technology is so fast it can capture exceptionally clear pictures of the heart while it is beating. This increased speed facilitates data acquisition from patients who have difficulty holding their breath, such as elderly or pediatric patients; and, it actually has the capability to capture thousands of images in a matter of seconds. SOURCE SOURCE (Service Options Using Resources in Community Environments) is a program developed by the state of Georgia and is funded by Medicaid. SOURCE is an enhanced case management program designed to provide elderly, disabled and low-income patients with a range of home and community based services assisting patients to remain in their home or community environment thus avoiding the need for nursing home placement, unnecessary emergency room visits and extended hospital stays. Most services are completely reimbursable through Medicaid at no cost to the patient. To be eligible for SOURCE, a patient must have Medicaid, receive SSI benefits and have a chronic medical condition such as diabetes or hypertension that requires monitoring by a physician. Community services available through SOURCE include: home delivered meals, respite for caregivers, emergency response system, personal support services (personal care/household chores), adult day health, coordination of home health and outpatient therapies and personal care homes. Case Managers will assess potential members in their home, determine what services are needed and develop individualized care paths for each member to reach their personal goals. Case Managers work closely with member's primary Care Physicians, outpatient centers, pharmacies, dialysis, and all provider services for coordination of care.
